hi Elisabeth,

Thanks for your question.

Regarding to uploading forms to the internet, you may need to consult your web deparment / colleagues to find out what channel (or the FTP address) does your company use to upload webpages.

You may save a Word document with a form as a webpage, then forward it to the web team.

Alternatively, you can create a form using a web authoring package: FrontPage or Dreamweaver. Then, upload the webpage onto the internet.
