Can different databases which have different field names but the same sort of data still have a relationship set up between them?

Yes it is possible to relate two tables that don't have exactly the same name of the primary key and the foreign key. More important factor is that the primary field and the foreign key should have the similar data type. If your Primary table has Auto number as a data type then the foreign key should also have either Auto number or number set as the data type.
If the Primary key is set as Text then the Foreign has to be set as text as well.
