Dear RachaelThank you for attending Upgrade to Office 2007 course!! I hope you enjoyed the course and benefited from it.
I am assuming that you know how to create forms in Outlook 2003.
In Outlook 20007 you simply have to choose Tools> Forms > Design a For and then choose the type of form you'd like to design and then save it.
Once you have designed a form then you can simply choose the form by choosing Tools> Forms > Choose a Form.
